Crossing the first threshold is when the hero enters the unknown. In my story, you might say the cave is the first threshold into the unknown or into some other place. There are examples of crossing the first threshold: the border of the shire from The Hobbit and The Lord of the Rings, the cantina from Star Wars, and Messina, Italy, from Kingdom of Heaven.
